COVID-19 Vaccines- Phase Two

The Hopkinton Health Department is utilizing this form to collect more information to help better determine exactly how many people fall into each priority group within our area. They will use the information collected to notify individuals that a local clinic is available in Hopkinton. Everyone should first continue to monitor the state vaccine map to find locations for vaccinations once their priority group is eligible. Additionally, you can sign up on the survey by which phase you are in so that the health department posted on Housewives and on Hopkinton Uncommon in order to be notified when the Hopkinton Health Department is going to have local clinics.
